How do you ween a baby from formula to milk?

ok so as i said before, i'm a first time mom, and her dr's seem to be no help these days always just trying to rush us out the door. so i wanted to ask other moms and dads, how do i ween my daughter from formula to milk, and at what age should i start doing this?
